What is at the heart of the increasing health care costs in America?
The question asks about increasing health care costs which is unrelated to health insurance and the associated gaming.

1. Governmental distortions to the market place via single payer practices by Medicare and Medicaid. These two programs and other like them cause price distortions which greatly benefit those covered by their programs at the expense of those who are not covered.

3. Government manipulation of procedures, methods, and best practices. Governments at all levels necessarily impose restrictions and requirements on medical service providers to... "protect the public".
4. Lack of profit initiative in most major medical centers. The vast majority of medical providers are operating in non-profit vehicles. This mmeans that there is no automatic incentive to cut, minimize, or avoid costs. Further, special allotments in rules and taxes for these non-profits allow them to just grow in size without reduction pressures. As long as someone will foot the bill, they will just spend.
5. Lack of price signaling available for the customer (aka the patient). Since the patient is not alerted to the price of a procedure before the procedure is rendered, the patient does not have the option to opt out or select alternative treatments. Further since the patient really doesn't have any ideas about the costs for procedures and because doctors try to right every time due to malpractice suits, more often than not, the doctor orders more analysis and procedures than effectively necessary. This overkill has to be paid for by someone.
6. Finally, moral hazard. As mentioned about since doctors don't pay unless things go wrong and there is lawsuit, they will order excessive procedures to protect themselves. LG
